Pre-K

Parents will need to provide proof of qualification by supplying the following documents:

1. Proof of meeting income guidelines by completing and submitting the Income Verification Application English or Income Verification Application in Spanish. You will also be required to provide your three most recent pay stubs for all working members of your household or your 2023 (filed tax 1040 form).

2.  Birth certificate.

3.  Immunization records (including Hepatitis A & B, DTAP, MMR, IPV & Varicella). Please contact your physician’s office to ensure compliancy for school.

4.  Proof of residency in Carson City (Utility bill, Mortgage or Rental agreement, Voter Registration, etc.).

5. Annual Student Health Update: Parents/Guardians are responsible for notifying the school if there is a new or existing health condition. This helps ensure your child receives safe and appropriate care at school. Please take the time to fill out the Annual Student Health Update form or in Spanish here.
